PLEASE READ THE FOLLOWING INFORMATION BEFORE REGISTERING YOUR CHILD

1. To register a child for the Faith Formation classes, please complete the following electronic forms.  PLEASE FOLLOW THE PROMPTS EXACTLY.  Failure to follow the prompts exactly may cause problems with registration.
2. Please complete only ONE form per student.  If you have multiple children in the program please come back to this page and click the Begin Registration Button below.
3. In addition to each registration, you will need to click the link to complete the Emergency Medical Form online.  Please complete this form as soon as possible after completing the registration form.  One form per child.
4. When you are finished filling out all Registration Forms, click the Online Payment Button below.
5. Information about programming schedules will be communicated and updated via email, once you have completed this form.

Child Faith Formation is offered on Sunday mornings:

St. Robert at 9:30 to 10:45
St. Thomas at 10:00 to 11:15
St. William at  8:30 to 9:45
